Name Change

If you need to make a name change on your account, we will need a written request that includes the following information: Owner name, owner number or numbers if you have more than one, previous name, new name, and your signature, in addition to the document identified below:

Marriage - Please send us a copy of your Marriage Certificate

Divorce or other - Please send us a copy of the divorce decree or other Order or legal document effecting the name change.

Business- Please provide a copy of the Certificate of Name Change or Merger effecting the change of the business name.
